Pharmaceutical companies report quarterly results through live webcasts, typically a prepared management presentation followed by an analyst question-and-answer session. These events are publicly accessible on company investor relations pages and through major financial data platforms. Following them requires specific literacy because GAAP financial headlines routinely misrepresent underlying performance when taken at face value.

Q2 2026 results from Pfizer, Johnson & Johnson, and Merck illustrate this pattern. All three delivered positive operational results and raised full-year guidance. All three also reported GAAP losses or large one-time charges that would confuse a reader scanning only the headline numbers.

Why GAAP Headlines Mislead in Pharma

Merck (MRK) reported Q2 2026 revenue of $16.6 billion, up 5% year over year (4% excluding currency effects), yet posted a GAAP loss of $0.13 per share. The source was a $5.7 billion charge related to the Terns Pharmaceuticals acquisition, which drove the effective tax rate above 160% for the quarter. That single transaction rendered the GAAP bottom line meaningless as a measure of ongoing profitability.

Pfizer (PFE) faced a similar dynamic. Q2 2026 revenue came in at $15 billion, up 1% operationally, while GAAP EPS registered at -$0.04. A $4.3 billion non-cash impairment charge tied to the sigvotatug vedotin Phase III trial failure erased reported profitability. Adjusted diluted EPS came in at $0.77. The impairment reflects a pipeline write-down rather than a cash outflow, which is exactly the kind of one-time item adjusted metrics are designed to strip out.

Reading Revenue Ex-Headwinds

Reported growth rates rarely tell the full story in pharma because drugs losing patent protection create a structural drag on the top line, even as newer products expand rapidly. Separating patent-cliff headwinds from the rest of the portfolio is a core skill in reading pharma results.

Pfizer's 1% operational revenue growth looks modest until COVID product contributions are excluded. Stripping those out, the underlying business grew 5% operationally. Pfizer has beaten EPS expectations for ten consecutive quarters and revenue expectations in nine of the last ten quarters, giving that underlying growth figure meaningful historical context.

Johnson & Johnson (JNJ) reported Q2 2026 quarterly sales of $25.3 billion, up 5.6% operationally. That figure absorbed a 460 basis point headwind from STELARA biosimilar competition, as lower-cost versions of the immunology drug began taking market share. Excluding that drag, the rest of the portfolio expanded at a faster rate.

J&J's oncology portfolio drove much of the offset. DARZALEX generated over $4 billion in Q2 2026, up nearly 18%. Newer combination therapies grew faster still: CARVYKTI rose 47.7%, TECVAYLI climbed 56%, and TALVEY gained 62.6%. Q2 adjusted EPS came in at $2.90.

Guidance as the Key Signal

Management teams typically set full-year guidance conservatively early in the year; a mid-year raise signals confidence that underlying trends are outpacing earlier assumptions. When a company raises guidance while absorbing a large one-time charge, the core business message is particularly clear.

Pfizer raised full-year 2026 revenue guidance to $60.5–$62.5 billion, up $500 million at the midpoint, while simultaneously cutting its COVID revenue forecast from $5 billion to $4 billion. Raising the total even as a key segment forecast fell implies the non-COVID portfolio exceeded internal plans.

J&J raised full-year 2026 operational sales growth guidance to 6.5%–7.1% and lifted adjusted EPS guidance to $11.50–$11.65. Merck raised full-year 2026 revenue guidance to $66.3–$67.3 billion and full-year EPS guidance to $2.66–$2.76, despite the Terns acquisition charge distorting Q2 GAAP EPS.

In a webcast, guidance updates are typically presented in a table near the end of the CFO's prepared remarks. Listening for the direction (raise, maintain, narrow, or lower) and the reasons offered is more informative than any single absolute range.

Pipeline Updates

Every pharma webcast includes a pipeline section covering clinical trial readouts, regulatory approvals, and commercial launch data. This section often carries the highest long-term significance, as a single Phase III failure can trigger billions in write-downs.

Pfizer disclosed that sigvotatug vedotin failed its Phase III trial, triggering the $4.3 billion impairment. Phase III failures represent the most expensive stage of development setbacks, coming after years of clinical trial spending. Webcasts typically describe the study design, the endpoint missed, and the company's next steps for the indication or the asset class.

Merck reported positive pipeline momentum. LIPFENDRA received FDA approval during the quarter. KEYTRUDA grew 4% to $8.4 billion. WINREVAIR grew 75% to $588 million; WELIREG rose 67% and CAPVAXIVE climbed 40%. A diversified commercial pipeline across multiple therapeutic areas reduces the risk any single product poses to overall portfolio performance.

J&J provided early launch data for ICOTYDE. Launch trajectory questions dominate the analyst Q&A for newly approved drugs, as prescription uptake in the first few quarters often predicts peak-year commercial performance.
